612 Fastpitch (MGFA Inc.) is committed to providing safe and healthy spaces for all our players, coaches, volunteers, and fans. To ensure we have a safe and healthy season, 612 Fastpitch has developed the following COVID-19 Plan in response to the COVID-19 pandemic. Our goal is to mitigate the potential for transmission of COVID-19 in our organization and communities, and that requires full cooperation among all of our partners.

Use this form to help us continuously improve our safety process. The form will allow you to upload documents or photos if it helps explain your idea. Thank you for helping to keep everyone safe!

IF YOU ARE SICK, PLEASE STAY HOME

Coaches, players, family members, or spectators who are sick or are experiencing any of the following symptoms or live with someone who is COVID-19 positive are not allowed to participate in 612-sponsored events. Symptoms include:

  • Cough
  • Shortness of breath or difficulty breathing
  • Fever
  • Chills
  • Repeated shaking with chills
  • Muscle pain
  • Unexplained Headache
  • Vomiting and/or Diarrhea
  • Sore throat
  • New loss of taste or smell
